Course Materials Order Form
If you order your course materials from Ed Map by mail or by fax, please fill out and submit the Ed Map Order Form.

Enrollment Extension
To request an enrollment extension, complete and submit the Enrollment Extension Form, along with the $55 extension fee, to our office.
Permission to Release Grades
If you submit a signed Permission Form to Release Grades allowing our office staff to do so, you may call our office, and our staff will tell you your grade over the phone.
Online Exam Request (for exams taken at Texas State University-San Marcos)
Students who live in or near San Marcos may take exams on the Texas State campus, at the Testing, Research-Support, and Evaluation Center (TREC). To schedule an exam, fill out the Test Request Form available online from the TREC website.
Off-Campus Exam Request Form (for exams not taken at Texas State University-San Marcos)
Follow the instructions in your study guide or in your TRACS course for submitting the Exam Request Form, enlisting a proctor, and scheduling an exam.
